Free Assessment 

Assessment is conducted by a Certified Gerontologist. We begin with an in-home assessment at no cost and no obligation. A casual and non-threatening social interaction. This approach is non-clinical and allows for immediate trust building. It is a conversation where we get to know each other with a gentle focus on your loved one.

The assessment includes an evaluation of:

  • Past and present medical history
  • Medications
  • Behavior concerns and memory loss
  • Emotional and mental health
  • Nutrition
  • Physical limitations
  • Safety of living environment
  • Hobbies and interests

Communication and follow up is the key....

  • We are available to meet with you to fully understand your needs and home environment and no cost with no obligation
  • Assist in developing the right plan of care for you and your loved one
  • Provide regular supervisory visits
  • Available access to a Care Manager 24/7


Your Care Manager will serve as a Liaison for you by providing you with additional trusted resources such as:

  • Local Community and Nationally recognized organizations
  • Home health
  • Physical therapy
  • Palliative care
  • Hospice
  • In-home safety devices
  • Elder law attorney/advance directives
  • Family counseling or mediation
  • Medical equipment
  • Emergency response systems
  • Medication dispensers and more

Needs may change at any time. We will be there to work with you and to re-assess any change to your plan of care before it becomes critical. No matter what changes occur, we can assist in making the proper recommendations that will benefit you and your family.


Geriatric Care Management is the process of planning and coordinating care of the elderly and others with physical and/or mental impairments to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible.

Our Care Managers accomplish this by combining a working knowledge of health and psychology, human development, family dynamics, public and private resources and funding sources, while advocating for their clients throughout the continuum of care.
